My name is Heather Hammack,
Heather Hammack is a local Family Nurse Practitioner. Born in laurel County, Heather experienced a simple and blessed childhood. Her grandparents owned and worked a dairy farm for many years, which allowed Heather to spend many days surrounded by so much of the beauty that Kentucky offers.
She is a graduate of North Laurel High School and an Alumni of both Somerset Community College and the University of the Cumberlands. Her education includes a Certification in Nursing Assistance, a Diploma of Nursing, an Associates of Applied Science, an Associates Degree in Nursing, a bachelor’s degree of Science in Nursing and a master’s degree of Science in Nursing, Family Nurse Practitioner. Her experience in healthcare includes intensive care, neurosurgical intensive care, medical-surgical, rehabilitation, home health, opioid and addiction rehabilitation, post-partum, utilization management, public health and family practice. She is certified by the American Association of Nurse Practitioners as an Advanced Practice Registered Nurse.
At a young age, Heather dreamed of becoming a nurse and caring for others. Throughout high school her goal was to enter a nursing program immediately after graduation. She persevered and pushed toward her goal of one day practicing in a family health clinic. Many years and a few degrees later, she is utilizing her expertise to provide that exact type of care to the families in rural Kentucky. Heather also serves as an adjunct Professor of Nursing at the University of the Cumberlands.
Heather, along with her husband and two children still reside in Laurel County. They enjoy serving at church in both worship and youth ministries. They enjoy camping, hiking and can be found jeeping the backroads of the state that they love!

I was raised in Jackson County, but I was born in Oak Ridge, Tennessee. My family moved to Kentucky when I was a child. I graduated from Jackson County High School in 2005. I am married to my husband, Jody of 14 years and have two children: Evan and Wyatt. I am a Certified Family Nurse Practitioner with dual certification in Psychiatric Mental Health across the lifespan and a certified Substance Abuse Disorder provider.
Prior to attending nursing school, I obtained an Associate's Degree from Somerset Community College in 2007 and worked as a Certified Medical Assistant. I then attended EKU for my Associates Degree in Nursing in 2010, obtained my Bachelor's in Nursing Degree from University of the Cumberlands shortly after. I then furthered my education at EKU in which I obtained my Master's of Science in Nursing with an emphasis in Family Practice in 2019. I obtained my Substance Abuse Treatment certification in 2020. I have been working as a Family Nurse Practitioner and SUD provider since.
I found while working as an FNP and SUD provider, that I wanted to be able to help provide more in depth care to my patients and to be able to develop a better understanding of those disorders. Therefore, I obtained my certification in Psychiatric Mental Health from EKU in May 2023. In addition, I have extensive training in insertion and removal of Nexplanon. In my free time, I enjoy going on walks and spending time with my family.
